The PSAL has also contacted us and told us that we will be offering some sports for students to participate in.

At this moment I do not know which sports will be offered. But I would like to be prepared.

When we go back to school will you play on a Sports Team?

If your answer is yes, you will need to complete the proper forms.

The Parental Consent Form will need to be filled out and signed by a parent or guardian.

The Medical Form will need to be filled out and signed by a Doctor.

Every student will need a new medical form because we have been out of school for a year and a medical-only last 1 year.
